Purchase of Packages of Multiple Sessions

Linda S. Greenfield at Empower Your Soul llc agrees to provide the client below with the pre-purchased sessions once client's payment is received, and to uphold the terms of this agreement.

Session appointments will be scheduled at mutually agreed upon days and times and be provided in person, via phone, FaceTime, or by using the meeting app "Zoom" for phones or tablets or via an internet web link to Personal Computers.

By completing and submitting this form (which will by emailed to Linda Greenfield/ Empower Your Soul), you, the client, are agreeing to all the terms and conditions stated on this form.

Contract Agreement Terms for Purchase of Session Packages

- I understand and accept that Linda S. Greenfield at Empower Your Soul llc agrees to provide me, the client, with the pre-purchased sessions once my payment is received, and will uphold all the terms of this agreement. - I understand and agree that my session appointments will be scheduled at future mutually agreed upon days and times and be provided either in person, via phone, FaceTime, or by using the meeting app Zoom for phones or tablets on via an internet web link on Zoom. - I understand and agree that I must use all prepaid sessions within one (1) year of purchase and any unused sessions will expired and become non-refundable after that time. - I understand and agree to give 24 hours of notice to cancel or reschedule any session appointments or I can be charged for any missed session(s) at Linda Greenfield’s discretion. - I understand and agree that I may cancel this agreement at any time in writing for any reason. - I understand and agree that in order to receive a refund for any unused prepaid package sessions, my written request to Linda Greenfield / Empower Your Soul llc is required. - I understand and accept that canceling this agreement also voids any special package discounted fee rates originally purchased. - I fully understand and accept that any refund monies owed to me for un-used prepaid sessions will be based on recalculations for the entire package using Linda Greenfield’s customary and usual non-package fee rate of $100 per session. - By completing and submitting this form, I am agreeing to all the terms and conditions stated on this form.
